Longevity has become one of the biggest buzzwords in health and wellness. You may see the word slapped on a label promoting certain supplements or linked to wellness practices such as cold plunges, peptide therapies, or the latest intervention promising to help you live as long as possible.

But with so much noise around longevity, it’s no wonder people are confused about what it actually entails. Here’s the truth: living a longer, healthier life isn’t about who has access to the latest and greatest technology, treatments, or tools.

Longevity also isn’t about stopping or reversing aging. It’s about preserving your physical and cognitive function, reducing your risk of chronic disease, and maintaining independence and quality of life for as many years as possible. In other words, the goal isn’t simply to add years to your life. It’s to make more of those years healthy.

At Biograph, that’s how we approach longevity medicine. Rather than chasing individual “anti-aging” treatments, we identify and measure the factors most likely to affect your long-term health, intervene where appropriate, and track how those factors change over time.

What does longevity mean?

At its simplest core, longevity refers to living a long life, but focusing exclusively on lifespan misses an important part of the equation: healthspan.

Lifespan is the total number of years you live, whereas healthspan refers to the years you spend in good health, without significant chronic disease or disability limiting your quality of life.

The World Health Organization (WHO) uses a similar framework in its definition of healthy aging, describing it as the process of developing and maintaining the functional ability that enables well-being as people grow older. [1]

Imagine two people who both live to age 90. One remains physically active, cognitively sharp, socially engaged, and independent until the final few years of life. The other spends the final 15 years managing multiple chronic diseases that substantially limit their mobility and independence.

Their lifespans may be identical, but their healthspans tell a very different story.

What’s the difference between longevity and anti-aging?

The two terms are frequently used interchangeably, especially in marketing, but they’re not the same thing.

“Anti-aging” implies that aging is a single, clearly defined phenomenon that we can stop. Not only is that impossible, it’s also not the end goal. Aging is a normal biological process, and no supplement, therapy, or wellness intervention can stop or reverse it. 

Longevity takes a more practical approach by focusing on the diseases and risk factors that can shorten healthspan and lifespan. Cardiovascular disease, metabolic dysfunction, osteoporosis, sarcopenia, and cognitive decline often develop gradually over years, with risk factors or early signs that can be identified well before they substantially affect your daily life.

We also know that many contributors to healthy aging are modifiable. You can improve your overall health by making various lifestyle changes, including:

  • Engaging in regular exercise

  • Eating nutritious foods

  • Getting enough quality sleep

  • Practicing stress management

  • Maintaining healthy blood pressure levels

  • Supporting metabolic health

  • Preserving muscle mass, strength and power

  • Avoiding tobacco

  • Staying up to date with preventive care

What factors influence longevity?

There isn’t one biomarker, behavior, supplement, or test that determines how long you’ll live.

Instead, healthy aging is influenced by a combination of factors, including hereditary and familial risks, environment, lifestyle, and personal medical history. The complex interactions among these elements is why a longevity strategy should look across multiple systems and their interconnections rather than focusing on a single number or intervention.

Genetics and family history

Genetics and family history

Genetics matter, but they don’t guarantee a particular health outcome.

Family history can help identify inherited vulnerabilities or shared exposures or behaviours which may increase the risks for conditions such as cardiovascular disease, certain cancers, diabetes, and neurologic disorders. 

However, many factors that may interact with these hereditary or familial risks and influence healthy aging remain modifiable. The National Institute on Aging highlights physical activity, nutrition, sleep, medical care, alcohol use, and mental health among the factors that can influence how we age. [2]

Your genetics may influence your risk, but they don’t eliminate the opportunity to change your health trajectory.

Cardiovascular, neurologic, and metabolic health

Cardiovascular, neurologic, and metabolic health

Cardiovascular and metabolic health are central to longevity because dysfunction in both can affect nearly every organ in your body.

Blood pressure, glucose regulation, insulin sensitivity, cholesterol and other lipid marker levels, inflammation, and cardiovascular fitness all interact to influence your long-term risks. For instance, insulin resistance can increase cardiovascular risk. High blood pressure can affect both your heart and brain. Chronic inflammation can impact cardiovascular and cognitive risks.

This is why looking at one “normal” cholesterol or blood glucose result doesn’t necessarily tell us whether someone is in good metabolic or cardiovascular health.

A more comprehensive assessment may include:

  • Advanced blood work

  • Scheduled blood pressure readings

  • Personalized cardiovascular screening

  • Brain and cardiovascular imaging when appropriate 

  • Other diagnostics based on your family history

Muscle, strength, mobility, and body composition

Muscle mass is one of the most important assets we can protect as we age.

Skeletal muscle supports glucose regulation, strength, balance, mobility, and the ability to remain physically independent. Yet muscle mass and strength naturally tend to decline with age, particularly without adequate resistance training and nutrition. [3]

Body weight alone doesn’t tell us whether someone has adequate muscle or where their body fat is stored, either.

A DEXA scan, for example, can measure lean mass, fat mass, visceral fat deposition, and bone mass. Measures of strength and movement quality provide additional insight into physical capacity.

Your cardiorespiratory fitness matters, too. VO2 max measures the amount of oxygen your body can use during maximal intensity exercise and is strongly associated with long-term health outcomes. [4]

Sleep, stress, and mental health

Sleep, stress, and mental health

Sleep is king, supporting your cardiovascular, metabolic, immune, hormonal, and cognitive function. Chronic sleep disruption can disrupt several of the same systems we’re trying to protect as we age.

Stress and psychological resiliency matter as well. Persistent stress can affect your sleep, blood pressure, glucose regulation, exercise habits, nutrition, and recovery.

A longevity strategy also has to account for diseases that may develop without obvious symptoms.

Preventive screenings allow clinicians to identify certain diseases or risk factors earlier, when there may be more opportunities to intervene. Which tests make sense depends on your age, sex, family history, medical history, exposures, and individual risk.

Advanced diagnostics can sometimes provide additional information, but more testing isn't automatically better.

What is longevity medicine?

Longevity medicine is a preventive approach to healthcare focused on identifying disease risk earlier and preserving health and function over time.

Rather than only asking whether someone has a disease today, longevity medicine asks: What can we identify and address before someone becomes sick?

That may involve evaluating metabolic biomarkers, cardiovascular fitness, body composition, bone mass, blood pressure, cognitive performance, sleep, and imaging findings. But testing alone isn’t longevity medicine. The information gathered from those screenings should help guide clinical decisions, targeted interventions, and ongoing monitoring.

How to start improving your own longevity

You don’t need an elaborate or expensive “longevity protocol” to support healthy aging.

The fundamentals remain the gold standard: regular aerobic and resistance exercise, nutrient-dense foods, adequate sleep, stress management, maintaining social connections, avoiding smoking, and staying up to date with preventive healthcare.

Personalization becomes valuable when determining which areas deserve the most attention for you. For instance, someone with above average aerobic capacity but lower muscle mass will require a different strategy than someone with moderate cardiorespiratory fitness but above average lean mass.

Instead of hopping aboard every longevity trend, consider gathering objective health data that can help identify your biggest opportunities for improvement and focus your efforts accordingly.

What tests are useful for longevity?

There’s no single longevity test. Depending on your age, health history, and risk factors, useful assessments may include blood biomarkers, blood pressure, body composition and bone mass, VO2 max and strength testing, cardiovascular screening, cognitive evaluation, and appropriate imaging. Testing is most valuable when the results can inform clinical decisions or guide a personalized prevention strategy.
